- Home Installation Agreement (HIA)
- Signed and uploaded to the lead on QuickBase under the "Contract Document File Attachment" section
- CPUC (Customer Protection Guide)
- Only required by PG&E, SCE, SDG&E, BVES, and PacifiCorp
- Requires customer Initial and signature
- The Sales Representative must upload the signed CPUC to the lead in the "Additional Document File" section if it is not included as part of the contract and is required under the utilities mentioned above.
- Utility Bill
- Provide a recent utility bill showing the following details
- Account holders name
- Service address (must match customer address from HIA
- Service Account Number
- Meter Number
- Rate Schedule
- Uploaded to the lead under QuickBase's "Utility Bill # Attachment" section.
- Please contact a Project Manager through our chat feature if you need to use a supplemental utility form. This form is not recommended and not acceptable for PPA programs.
- Provide a recent utility bill showing the following details

- HOA approval waiver
- Homeowner consent to deliver and set up the solar system while the written approval of the HOA is pending

- As part of the welcome call, ESP's Project Managers will verify whether the homeowner is associated with a homeowners' association (HOA) and identify the individual responsible for handling the application.
- If the homeowner or the Sales Representative has processed the HOA application and received approval, they can reach out to ESP support, providing the HOA documents, to get them filed within the project in QuickBase.
- ESP HOA processing is available if the adder is included.
- As part of the welcome call, ESP's Project Managers will verify whether the homeowner is associated with a homeowners' association (HOA) and identify the individual responsible for handling the application.
- Layout for Homeowner
- If there was a discussion about layout during the sales process, the sales representative should upload a document demonstrating the layout to the lead on QuickBase in the "Layout Present to Homeowner File Attachment" section.
